District Assembly Forms
2009-2010 

Most of these forms are available in Adobe Acrobat and Microsoft Word format.  You will need one of these programs. If you are going to use Adobe Acrobat Reader, the Reader and updates are free to download from www.adobe.com.  You will need Adobe Acrobat Reader version 8 at least. You will be able to fill out the forms on the computer and print them from Adobe; those with “Submit by Email” at the top can be emailed. Please note:  You will not be able to save the files in Adobe. You will only be able to print the form (You must use the action buttons [“Print”], if they appear at the top of the page, to print Adobe forms).  Please proof the form before you close the file. If you choose to use the Word format you will be able to fill in, print, and save these files. But please remember to change the name of the file or you will lose the original form. Once saved, these can be attached to an email and sent to office@dallasnaz.org.  Some of these forms must be signed. DO NOT type your name on the Signature line.  If you choose to email the forms to us you can sign them at Assembly.  Otherwise, please sign them and mail them to the District Office at P.O. Box 1099, Rowlett, TX 75030.

PASTORS, the Annual Pastor's Report is only a worksheet for you to print and use when filling out your report online.  To submit your report, you must go to https://apr.nazarene.org or https://secure.nazarene.org/nazid using the Nazid user account that you have been registered for.  If you are having problems logging on, please contact the District Office.
